There is very little information you can gather about a SQL server via SNMP beyond monitoring the SQL processes and services. To monitor SQL using something like AppInsight for SQL, you will need to open the SQL server ports those instances are listening on through the firewall so Orion can access them. By default MSSQL uses TCP Port 1433, but as you have multiple instances configured it's possible your SQL server is configured to use dynamic ports or multiple fixed TCP Ports. You will need to consult with your DBA or your SQL Servers network configuration to determine which ports need to be opened on your firewall to allow SAM to access those instances.
